no problem open your pdf on photoshop and do slice and after that save as html format

Yeah. I just realized it's almost impossible to convert this file that I'm working with. It's a government form. They have max security for this. Thanks for the help tho guys

Well, then screenshot or extract images and embed images on HTML. Or do you want the text to be selectable?

This form is interactive; has check boxes and blanks to fill up by patients. Everything i try, keep getting messages "the author doesn't allow this or that". There are only two options left, i have to recreate the exact form or contact our officials. Thanks again

you can also convert the pdf to text document and write html code or code the whole document with html.. anything is possible...
